Application Process

Families are selected to participate based on need, rather than on a first-come-first-serve basis. To apply, you will need to submit a childcare application along with documentation that you meet the requirements for eligibility.

BEFORE YOU APPLY

Before you complete a childcare application, please gather the following documents to determine eligibility for the Early Head Start program.

  • Proof of Identity – Photo ID (Driver’s License, Passport, Military ID, etc.)
  • Proof of Residence – Current residence in the service area (Lease, Utility Bill, Mortgage Statement, etc.)
  • Proof of Age of child enrolling (birth certificate or hospital documentation proving live birth signed by a hospital official)
  • Proof of Income (Pay stubs, Receipt of Child Support, TANF [Cash], SSI, Educational Scholarships and/or Grants, etc.):
  • Insurance Information (Medicaid, Private Insurance, etc.)
  • Disability/Part C Verification (if applicable)
  • Eligibility Determination Documentation: Individual Family Service Plan (IFSP) or Foster Care Placement, Adoption, or Legal Guardianship Verification (if applicable)
  • Homeless Verification (if applicable)
  • Referral Verification and Proof of Services (Food Stamps, WIC, Florida Kid Care, Subsidized Child Care, etc.)

STEP 1: Complete a Childcare Application

You can also apply for our Early Head Start program at any one of our four locations. Simply download this childcare application, fill it out and drop it off during business hours along with documentation of your eligibility requirements.

AFTER YOU APPLY

Applications are typically reviewed within three business days. To find out the status of your application call 850-414-9800 or contact our team. If your child’s application is accepted, you will be notified by a family advocate at the preferred email or phone number provided in your application.
